1. Collect 6 to 15 corks from bottles of Mad Housewife wine.
2. Download, print, and completely fill out the “Collect Corks Giveaway” entry form: PDF Download.
3. Mail the completed entry form, along with the correct number of corks, to the address on the entry form.
4. Allow 6 to 8 weeks for delivery of your free gear!
5. Lather, rinse and repeat as many times as you like for all the fabulous Mad Housewife swag.

6 corks = Hat
12 corks = Gym Bag
